More and more populations have health problems, the resolution of which does not depend only on the performance of the health system. For example, aspects such as sedentary lifestyle, inadequate eating habits, social isolation and loneliness, economic difficulties, discrimination, have a profound impact on health and well-being and require multisectoral responses, in an approach to integrating person-centered care.
Social Prescribing is an innovative approach that allows you to create links between people, health care and existing support resources in the community to improve your health, well-being and quality of life.
In this sense, the National School of Public Health of NOVA University of Lisbon (NOVA NSPH), motivated by the growing need to generate health evidence, activate resources for health and disease management and co-create solutions with social and health impact, with the involvement of people, organizations and communities, develops the Social Prescribing Portugal knowledge center, in order to boost Social Prescribing in the national context.
Social Prescribing Portugal assumes a relevant role at the national and international level, acting on three axes:
Social Prescribing arises from a real need of health professionals, when they are confronted with social, emotional and practical challenges of users that impact their health and well-being, and exclusively clinical approaches are not enough to respond in a preventive and health promotion aspect.
In a model of integrating health, social and cultural care, Social Prescribing facilitates the approach of health care users to existing support resources in the community through individualized responses adapted to the specific needs of each one. The answers can be promotion of healthier lifestyles (e.g. physical activity, healthy cooking), contact with nature (e.g. walking, gardening), socioeconomic support, social activation, personal and professional development (e.g. volunteering, training and employment), and cultural participation (cultural, artistic and creative activities), among many others.
In this way, Social Prescribing is an opportunity to strengthen the articulation between different sectors and actors to effectively solve the non-clinical social needs of each person.
Social Prescribing was born in the context of primary health care in the United Kingdom, in the late 1990s, and has since developed reaching new territories and in several modalities. The significant expansion of Social Prescribing at the global level has been accompanied by the World Health Organization. It is currently consolidated in more than 20 countries. In Portugal, the implementation of Social Prescribing began in 2018, in the pilot project in Family Health Units in Lisbon. NOVA NSPH participated, from the beginning, in the planning and implementation of this project, and developed its monitoring and evaluation. Currently there are numerous Social Prescribing initiatives to be developed, from the north to the south of the country, with the support of the School, in partnership with several health units, municipalities and civil society organizations.

The Social Prescribing Portugal team is composed of multidisciplinary specialists, with extensive experience in training and skills reinforcement, research, development, monitoring and evaluation of interventions focused on acting on the social determinants of health, with a view to improving the health of populations.
We intervene through collaborative approaches, actively involving several multisectoral partners to generate evidence in the area of Social Prescribing and co-create, implement and evaluate innovative and inclusive strategies and solutions with significant positive impact.
At Social Prescribing Portugal we are committed to designing, developing and implementing projects adapted to the specificities of each context in terms of scope, approach, recipients and governance models, through excellent technical and scientific support.
Our partner network is constantly expanding, which allows us to accumulate experience to help other teams implement Social Prescribing in their contexts.
Our team also develops conceptual, technical and methodological resources (e.g. Social Prescribing – Manual to support the implementation of initiatives, reports, scientific articles) in order to support the continuous improvement of Social Prescribing initiatives and strengthen their efficiency, effectiveness and sustainability, based on the best scientific and empirical evidence available.

Implementation support – Technical and scientific support in the continuous improvement of existing projects and in the implementation of new Social Prescribing projects at the local, regional and national levels.
Monitoring and evaluation – Design and development of studies for monitoring and evaluation of Social Prescribing.
The Portugal Social Prescribing Network is promoted by NOVA NSPH and has the institutional support of the Executive Directorate of the SNS, Portugal Inovação Social, the Instituto da Segurança Social, the Direção-Geral da Saúde, the Comissão de Coordenação e Desenvolvimento Regional de Lisboa e Vale do Tejo, I.P., the Plataforma Saúde em Diálogo and the Fórum Saúde XXI.
The Network integrates researchers, professionals from various areas, service users, managers and policy makers, and intends, in a collaborative approach, to enhance skills, share experiences and disseminate strategies for the effective and sustainable implementation of Social Prescribing in Portugal.
