Social Prescribing has emerged as an innovative approach to health and social, person-centered care integration. Its main objective is to prevent the disease, promote health and well-being, as well as assist in the management of the disease itself. Social Prescribing seeks to respond to social, emotional and practical needs that impact health, using community resources as a complement to clinical or pharmacological responses.

 

THE COURSE

 

The Social Prescribing: from Theory to Practice course is the first developed in Portugal and aims to expand knowledge and strengthen the skills of professionals from different sectors for the planning and implementation of initiatives. Its approach is based on evidence and best practices identified at the international level.

Participants will develop a clear understanding of Social Prescribing, its relevance and its benefits. The course will address the roles of the different stakeholders and the challenges and opportunities for the implementation of initiatives.

In the end, participants will be prepared to apply the concepts acquired in their professional contexts, contributing to a more holistic approach in responding to the needs of users. The course also aims to support the planning of new initiatives and strengthen existing ones.
