
CONCORDIA HF – Assessment of the main concerns of individuals with heart failure in disease management, impacts on daily life, key barriers to access, and the most valued factors in healthcare

The CONCORDIA HF project aims to: (i) design a disease progression staging segmentation model focused on the study’s objectives, (ii) identify the main barriers to healthcare access for individuals with heart failure (HF); (iii) investigate the existence of perception differences, from the perspective of healthcare professionals and individuals with HF, regarding the main concerns in HF management, the impacts of the disease on daily life, and the most valued factors in healthcare. It also aims to design and assess the feasibility of a set of action strategies to align the healthcare professional’s perspective with that of the individual with HF, contributing to the alignment of information provided in healthcare settings, and to develop resources for promoting health literacy in HF.
